I understand that any omission or misstatement of material fact on this application or on any document used to secure employment shall be grounds for rejection of this application or for immediate discharge if I am employed, regardless of the time elapsed before discovery.

BACKGROUND CHECKS:

If considered for a position, I hereby authorize the Company to thoroughly investigate my references, work record, education and other matters related to my suitability for employment and, further, authorize the references. In addition, I hereby release the Company, my former employers and all other personals, corporations, partnerships and associations from any and all claims, demands or liabilities arising out of or in any way related to such investigation or disclosure.

DRUG SCREENING:

I understand that as part of the Company’s employment process, if I am offered employment, I may be required to satisfactorily complete a drug screening test that detects drug use over an approximate period of a few months before I am eligible to become employed by the Company. The test will be administered at the Company’s expense, and will require me to provide a urine specimen for analysis. The specimen will be analyzed for the presence of cocaine, amphetamines, marijuana, opiates and phencyclidine (PCP). Results of the drug test are confidential and will not be disclosed to others without my specific written consent.

PROOF OF AUTHORIZATION TO WORK IN THE UNITED STATES:

If I am offered a position with the Company, I agree that on my first day of employment, I will bring documents verifying my legal authorization to work in the United States. (Federal law requires that you provide proof that you have applied for the required documents within your first three (3) days of employment and that you provide the actual documents within the first ninety (90) days of employment).

GENERAL:

I understand that I am required to abide by the policies and guidelines established by the Company.

I understand that nothing contained in the application, or conveyed during any interview that may be granted or during my employment, if hired, is intended to create an employment contract between me and the Company.
